Note: the form can be filled in online, then printed for signing and mailing.

If you wish to expedite the credit check process, you may fax a copy of the completed form to SpecTech, Inc. at 952-9322942.
Orders can also be expedited by paying for you initial order by credit card. We accept Visa, Master, and Discover.
You will still need to mail the signed form to the address above.
